Heel pain and plantar fasciitis treatment in San Antonio TX by Dr. Andrew Gunter DPM

What is plantar fasciitis?

Plantar fasciitis is the most common cause of heel pain and the single most common condition seen in a podiatry office. It occurs when the plantar fascia — a thick band of connective tissue that runs along the bottom of the foot from the heel bone to the base of the toes — becomes inflamed or irritated.

The plantar fascia functions as a shock absorber and supports the arch of the foot during walking and standing. When it is subjected to excessive tension or stress, small tears develop in the tissue. The body's inflammatory response to those tears produces the characteristic heel pain that plantar fasciitis patients experience.

The hallmark symptom is sharp, stabbing pain on the bottom of the heel that is worst with the first steps of the morning or after a period of rest — and typically improves after a few minutes of walking as the tissue warms and stretches. Pain may return after prolonged standing or activity.

Plantar fasciitis does not resolve on its own without addressing the underlying mechanical cause. Dr. Gunter evaluates not just the inflammation but the biomechanical factors contributing to it — foot structure, gait pattern, footwear, activity level, and body mechanics — to build a treatment plan that addresses the root problem rather than managing symptoms indefinitely.

Treatment options

Dr. Gunter takes a conservative-first approach to plantar fasciitis — beginning with the least invasive effective treatment and progressing only when needed. The right treatment depends on the severity of your condition, how long you have had it, and the underlying biomechanical factors contributing to it.

Orthotics / Arch Supports

Prescription foot orthoses designed specifically for your foot structure and gait mechanics. Unlike generic insoles, custom orthotics correct the underlying biomechanical cause of plantar fasciitis — redistributing load, controlling pronation, and reducing stress on the plantar fascia with each step. One of the most durable long-term solutions available.

Targeted stretching protocols

A structured program of plantar fascia and calf stretching — prescribed and supervised by Dr. Gunter — to reduce tension on the fascia, improve flexibility, and address the mechanical contributors to inflammation. Stretching programs are individualized based on your specific presentation.

Corticosteroid injection

A precisely placed in-office injection of corticosteroid medication to reduce acute inflammation and provide significant pain relief. Most effective when used as part of a broader treatment plan rather than as a standalone solution.

Shockwave therapy

A non-invasive treatment that delivers acoustic energy to the affected tissue, stimulating the body's natural healing response and reducing chronic inflammation. Effective for cases of plantar fasciitis that have not responded adequately to other conservative treatments. Typically done in conjunction with physical therapy.
