Understanding Ankle Arthritis

Ankle arthritis — degenerative joint disease of the tibiotalar joint — is less common than hip or knee arthritis but produces equally significant pain and functional limitation when advanced. Unlike the hip and knee, where primary osteoarthritis is most common, the majority of ankle arthritis is post-traumatic — resulting from prior ankle fractures, chronic ankle instability, osteochondral defects, or other injuries that damaged the cartilage surfaces of the joint over time.

 

The ankle joint has relatively thin cartilage compared to the hip and knee — typically one to two millimeters — and this cartilage has limited capacity for self-repair. Once damaged by trauma, chronic abnormal loading from instability, or direct articular injury, the cartilage deteriorates progressively. The joint space narrows on imaging, bone spurs develop at the joint margins, and the inflammatory response to cartilage breakdown produces the swelling and pain characteristic of established arthritis.

 

The most common cause of ankle arthritis in active and middle-aged adults is prior ankle fracture — particularly fractures that were incompletely reduced or that involved the joint surface. Even a fracture that was well managed can produce gradual post-traumatic arthritis over years if the articular cartilage was damaged at the time of injury. Chronic ankle instability is the second most common precursor — repeated abnormal joint loading from ligament laxity causes cumulative cartilage damage that becomes symptomatic as degeneration advances.

 

Symptoms of ankle arthritis include deep aching or throbbing pain in the ankle joint — particularly with weight bearing and activity — morning stiffness that improves after a few minutes of walking, swelling that worsens throughout the day, and progressive loss of range of motion. The pain pattern tends to improve with rest in earlier stages and becomes more constant as the disease advances.

Total ankle replacement preserves motion and has improved significantly in recent decades for appropriate candidates.
