An ankle sprain is one of the most common musculoskeletal injuries — and one of the most commonly undertreated. The widespread belief that a sprained ankle simply needs rest and time is responsible for a significant number of patients who develop chronic ankle instability that limits their activity for years after the original injury.
A sprain occurs when the ankle rolls inward — the most common mechanism — stretching or tearing the lateral ligaments on the outside of the ankle. The severity ranges from a mild stretch with minimal structural damage to a complete ligament tear with significant instability. All three grades of sprain can produce similar levels of pain and swelling in the acute phase, which makes clinical evaluation important — the appearance of the injury does not reliably predict its severity.
When a sprained ankle is not fully rehabilitated, the stretched or torn ligaments may not regain their original tension and structural integrity. The result is chronic ankle instability — a condition where the ankle feels loose, rolls easily on uneven ground, and re-sprains with progressively less force and provocation. Each subsequent sprain causes additional ligament damage, worsening the instability and increasing the risk of cartilage injury within the joint over time.
