What Is an Ingrown Toenail?

An ingrown toenail occurs when the edge or corner of a toenail grows into the surrounding skin rather than over it. The result is pain, redness, swelling, and — if left untreated — infection. The big toe is most commonly affected, though any toe can develop an ingrown nail.

 

Ingrown toenails develop for several reasons: improper nail trimming technique (cutting the nail too short or rounding the corners), tight or narrow footwear that compresses the toes, nail trauma, or an inherited tendency toward curved nail growth. In some patients the nail plate is naturally curved or wide, making ingrown nails a recurring issue regardless of footwear or trimming habits.

 

Mild ingrown toenails produce localized tenderness and redness at the nail border. More advanced cases develop soft tissue overgrowth around the nail edge, drainage, and frank infection — characterized by increased redness, warmth, swelling, and purulent discharge. Infected ingrown toenails require prompt treatment.

 

For patients with diabetes, peripheral vascular disease, or peripheral neuropathy, ingrown toenails require urgent evaluation regardless of apparent severity. Reduced sensation means injury can progress significantly before it is felt, and impaired circulation means infection can spread and wounds can fail to heal. What appears to be a minor nail problem in a diabetic patient can become a serious threat to limb health without prompt attention.

Ingrown Toenail Treatment Options

Most ingrown toenails are resolved in a single in-office visit. Dr. Gunter evaluates the severity and recommends the most appropriate approach for your specific situation.

In-office nail border removal

The most common treatment for a painful ingrown toenail. Under local anesthesia, Dr. Gunter removes the offending nail border — the portion of the nail that has grown into the skin. The procedure typically takes less than 30 minutes, provides immediate relief, and most patients return to normal footwear and activity within days.

Permanent nail border correction (matrixectomy)

For patients who experience recurring ingrown toenails in the same location, permanent correction prevents regrowth of the offending nail border. A chemical or surgical matrixectomy removes a narrow portion of the nail matrix — the tissue responsible for nail growth — so the problematic nail edge does not regrow. The visible nail width is minimally affected.

Infection management

When an ingrown toenail has developed a significant infection, Dr. Gunter addresses both the nail and the infection — providing nail border removal and appropriate antibiotic management when indicated. Attempting to manage an infected ingrown toenail at home without professional treatment typically prolongs the infection and increases the risk of it spreading.
