Professional Foot Care for Patients Who Need It Most — and Those Who Simply Prefer It
Routine nail trimming, callus management, and foot skin care are straightforward tasks for most healthy adults. For many patients, however, self-care is difficult, risky, or simply not possible — and professional podiatric management of these routine needs is both appropriate and, in qualifying cases, covered by Medicare and most major insurance plans.
There are two distinct groups of patients this service is designed for.
The first group includes patients with systemic conditions that make foot self-care a medical risk — particularly those with diabetes, peripheral vascular disease, peripheral neuropathy, or other conditions that impair circulation or sensation in the feet. For these patients, a nick from improper nail trimming, an unrecognized callus developing under a pressure point, or a skin break that goes undetected can progress to serious infection, ulceration or gangrene. Professional nail and skin care by a podiatrist who is also monitoring the full foot at every visit is a genuine component of comprehensive medical care for these patients — and Medicare and most major insurers recognize it as such for patients with qualifying diagnoses.
The second group includes patients who do not have a qualifying medical condition but who find professional podiatric nail and skin care preferable to self-care or salon services — whether because of difficulty reaching their feet, thick or dystrophic nails that require professional instrumentation, or simply a preference for care delivered in a clean clinical environment by someone who is also looking at the whole foot. For these patients, nail and skin maintenance is available as a cash pay service at a straightforward, clearly communicated rate.
Medicare Coverage for Routine Foot Care:
Medicare Part B covers routine nail care and foot skin care for patients with documented qualifying systemic conditions that make self-care a hazard. Qualifying conditions include:
• Diabetes mellitus (with specific clinical findings)
• Peripheral vascular disease
• Peripheral neuropathy
• Chronic thrombophlebitis
Coverage requires documentation of a qualifying diagnosis in your medical record from a primary care physician or other specialist. Routine foot care for patients without a qualifying condition is not covered by Medicare. We do offer it as a cash service for non-covered patients.

Nail & Skin Care Services
Nail trimming & debridement
Professional trimming and debridement of toenails — including thickened, dystrophic, or difficult-to-manage nails that require clinical instrumentation. Each visit includes inspection of the nail and surrounding skin for early signs of ingrown nails, fungal infection, pressure injury, or other changes requiring attention.
Callus & corn debridement
Professional reduction of calluses and corns using appropriate clinical instrumentation — more precise and thorough than home management, and carried out with inspection of the underlying skin for tissue breakdown or pressure injury. Particularly important for diabetic patients where callus buildup can conceal early ulceration.
Foot skin assessment & care
Each maintenance visit includes a comprehensive visual assessment of the skin surface — checking for early skin breakdown, maceration between the toes, early signs of infection, and pressure points that may require orthotic or footwear intervention. Skin care guidance specific to the patient's foot type and health status.
Diabetic foot surveillance
For diabetic patients, routine maintenance visits serve a dual function — providing the nail and skin care the patient needs while also performing the periodic foot surveillance that is a standard component of diabetic foot management. Vascular and neurological status is assessed at appropriate intervals. Dr. Gunter coordinates findings with the patient's primary care team when indicated.

Professional Foot Care Without a Qualifying Diagnosis
Patients who do not have a qualifying diagnosis for insurance coverage are welcome to receive nail and skin maintenance care as a cash pay service. Professional podiatric nail and skin care differs meaningfully from salon services — it is performed in a clinical environment with sterile instrumentation, by a podiatrist who is simultaneously assessing the foot for any changes requiring medical attention.
Common reasons patients choose cash pay podiatric maintenance care include difficulty reaching or seeing their feet comfortably, toenails that have become too thick or curved to manage safely at home, a preference for the clinical oversight that comes with every podiatric visit, and concerns about the hygiene standards of non-clinical nail care environments.
